Fiscal Year 2008 Forms and Information
- Data Collection Form
This spreadsheet will help you collect the necessary data on each of your sites and compile it automatically. When you open it, look at the bottom tabs. There is a Help Page, a page for each individual site, and a Tally Page. Formulas are built into the last page(the Tally Page) and will automatically carry your information over to be used for reporting. To make your trimester reporting easier and more accurate I would suggest (this is just a suggestion) that you ask your Site Coordinators to fill this out every month and send it to you via email. Then when you do your reports in March and in July, you will be able to look this over and cut and paste the information into your report, edit and format it. This would also assist you in working with your different sites to make changes on areas that are not being addressed. At meetings you could brainstorm ideas for implementing additional strategies, etc. If you have questions about this form please call or email Joan.
